Registration Instructions

How to Register:
Choose from the following two options:

Pay by Credit Card
Complete the following interactive online form. Once completed press SUBMIT to be redirected to PayPal to complete payment transaction. You will receive an email notification confirming that your registration has been submitted and payment has been processed.

Pay by Check
Complete the following interactive online form. Once completed press SUBMIT to be redirected to a confirmation page. Print off this page and mail-in with check made payable to The American Society for Metabolic & Bariatric Surgery. You will receive TWO email notifications: (1) confirmation that your registration has been submitted and is awaiting payment; (2) confirmation that your payment has been received in the ASMBS office.

The Annual Meeting is open to the following:
(1) All ASMBS members, (2) any surgeon/physician non-member, (3) any Integrated Health non-member who is an invited guest of an ASMBS member. ASMBS members may invite up to two non-members. The name of the inviting member must be provided on the online registration form.
Please Note:  Only the person registered will be allowed to pick up meeting materials. Please have picture identification available.

PAYMENT
In order to be processed, the registration form MUST be accompanied by payment. Acceptable forms of payment include check/money orders and credit cards (MasterCard, Visa, American Express). Checks should be made payable to the American Society for Metabolic and Bariatric Surgery.
If you do not receive an email confirmation after seven business days of registration submission, please contact the ASMBS office at 352.331.4900.
On-site registration will be available Sunday – Wednesday from 7:00am – 5:00pm and on Friday, 7:00am – 1:00pm. On-site registration is on a space-available basis.

CANCELLATION AND REFUND POLICY
Cancellation and refund requests must be submitted in writing and postmarked on or before May 23, 2012. Refunds requested on or before May 23, 2012 will be granted minus a $50.00 administrative fee.  Refunds will not be granted to no-shows and/or changes in registration made after May 23, 2011. No refunds will be granted after May 23, 2012.

COURSE INFORMATION
Postgraduate Courses | Sunday – Tuesday

  • Registration fees for these courses include the following: instruction, refreshment breaks, and lunch in the Exhibit Hall on Tuesday.
  • Welcome Reception and “ASMBS AfterDark” tickets are NOT included with registration for these courses, with the exception of the Integrated Health Main Session in which a complimentary Welcome Reception ticket is provided. Complimentary ticket must be requested at the time of registration.
  • Please note that tickets will be required for entry to both the Welcome Reception and “ASMBS AfterDark”.
  • There is no charge for admission to the Industry-Sponsored Symposia for Annual Meeting attendees.

Plenary Session | Wednesday – Friday

  • Registration fees include the following: instruction, refreshment breaks, lunch in the Exhibit Hall (Wednesday and Thursday), one complimentary ticket to the Welcome Reception (Tuesday) and one complimentary ticket to “ASMBS AfterDark” (Thursday). Complimentary ticket must be requested at the time of registration.
  • Guest tickets may be purchased separately for the Welcome Reception and “ASMBS AfterDark”.
  • Please note that tickets will be required for entry to both the Welcome Reception and “ASMBS AfterDark”.
